Consent to the processing of personal data for marketing purposes

By ticking the consent box, I express my consent to the processing of my personal data for marketing purposes of the Blažek Club loyalty program and inclusion in the customer database established and managed by BLAŽEK PRAHA as, Company ID: 25624407, with its registered office in Prague 10 − Hostivař, U Tóváren 261, ZIP code 102 00, entered in the Commercial Register kept by the Municipal Court in Prague, Section B, File 12599, email: info@blazek.cz (hereinafter referred to as "BLAŽEK PRAHA as"), as the administrator of these personal data. The processed data includes personal data within the scope of the registration form, records of movements on the BLAŽEK PRAHA as website (so-called cookies), data on orders and purchases. I agree to the possibility of using manual and automated processing, on the basis of which it may be decided to create a personalized offer of products or services, send commercial communications, promotional or advertising materials or invitations to events organized by BLAŽEK PRAHA as by e-mail, postal mail, telephone or SMS. I grant this consent for the duration of my membership in the Blažek Club and after termination of membership for the period strictly necessary for the purpose of protecting the legitimate interests of the administrator and for settling relations arising from the termination of membership (archiving, etc.).

Consent is voluntary and can be revoked at any time electronically on the website of BLAŽEK PRAHA as, at the e-mail address dpo@blazek.cz or in writing at the correspondence address of BLAŽEK PRAHA as, Poděbradská 538/46, 190 00 Prague - Vysočany.

The receipt of electronic commercial communications can be canceled via the link contained in each individual commercial communication.

BLAŽEK PRAHA as, as the controller, also processes personal data through its processors, and I agree that BLAŽEK PRAHA as is authorized to provide my personal data to these processors to the extent strictly necessary for the specified purpose of processing. Processors according to the previous sentence are understood in particular to be persons entrusted with the administration of the personal data database or ensuring its technical solution. Rights in connection with the processing of personal data

In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation), you have the right to request information about the purpose of the processing of personal data, information about the scope of the personal data that are the subject of the processing and their categories, including information about the source from which the personal data were obtained, the planned duration of the processing, whether automated decision-making, including profiling, is taking place, and information about any recipients or categories of recipients of the personal data. You also have the right to request the correction or deletion of your personal data if they are inaccurate or if the purpose of their processing no longer applies. If personal data are transferred to a third country, you have the right to be informed of this, as well as of the appropriate security safeguards that apply to the transfer.

You also have the right to request the transfer of all personal data concerning you that you have provided to BLAŽEK PRAHA as based on your consent or for the purpose of concluding a contract. Upon your request, we will provide you with this information without undue delay in a structured, commonly used and machine-readable format. This right does not apply to personal data that is not processed automatically.

If you believe that the processing of your personal data is in conflict with the protection of your private and personal life or in conflict with the relevant legal regulations, you are entitled to object, request explanation and elimination of the situation thus arising, in particular, to request restriction of processing, correction, supplementation or destruction of personal data.

Consent to the processing of personal data for the purpose of registration

By ticking the consent box, I confirm that my contact details provided in the registration form are correct, and I also express my consent to the processing of my personal data for the purpose of inclusion in the customer database established and managed by BLAŽEK PRAHA as, Company ID: 25624407, with its registered office in Prague 10 − Hostivař, U Tóváren 261, ZIP code 102 00, entered in the Commercial Register kept by the Municipal Court in Prague, Section B, Insert 12599, email: info@blazek.cz (hereinafter referred to as “BLAŽEK PRAHA as”), as the administrator of this personal data. Personal data is processed manually and automatically. The processed data includes personal data within the scope of the registration form and data generated by membership in the Blažek club loyalty program, records of movements on the Blažek website (so-called cookies), data on orders, purchases and information on payments. I grant this consent for the duration of my membership in the Blažek Club and after termination of membership for the period strictly necessary to protect the legitimate interests of the administrator and to settle relations arising from the termination of membership (archiving, etc.).

I acknowledge that by granting this consent and for the duration of its validity, I become a member of the Blažek Club and can benefit from all the benefits resulting from it.
