Information memorandum of BLAŽEK PRAHA as

Dear customer,

allow us to inform you about how we process your personal data at BLAŽEK PRAHA as. The purpose of the Information Memorandum is to provide you with information, in particular, about what personal data we collect, how we handle it, from what sources we obtain it, for what purposes we use it, to whom we may provide it, where you can obtain information about your personal data that we process, and what your rights are in the area of personal data protection. The company BLAŽEK PRAHA as, Company ID: 25624407, with its registered office in Prague 10 − Hostivař, U Tóváren 261, ZIP code 102 00, entered in the Commercial Register kept by the Municipal Court in Prague, Section B, Insert 12599 (hereinafter referred to as the "company BLAŽEK PRAHA as"), which is the operator of a network of its own establishments (hereinafter referred to as the "Blažek brand stores" or individually as the "Blažek brand store") and an online store (hereinafter referred to as the "online store") on the website www.blazek.cz, is, as the controller of personal data, obliged to protect your personal data and comply with the obligations arising from generally binding legal regulations concerning the protection of personal data.

1.Personal data

BLAŽEK PRAHA as obtains personal data of customers within the scope of the order form and the registration form for the Blažek Club, available either online on the website of BLAŽEK PRAHA as, or in the printed form of the application for the Blažek Club, available in Blažek branded stores.

2. Blažek's authorization to process personal data

BLAŽEK PRAHA as is authorized to process personal data based on the consent granted in order to ensure the proper functioning of the Blažek Club, based on a concluded contract in order to fulfill the rights and obligations of BLAŽEK PRAHA as towards customers (such as data on orders, purchases and information on payments) and the legitimate interests of BLAŽEK PRAHA as (such as processing personal data for statistical purposes, informing customers about the activities of BLAŽEK PRAHA as, records of movements on the BLAŽEK PRAHA as website (so-called cookies), preventing hacker attacks, etc.), for the duration of membership in the Blažek Club and after termination of membership for the period strictly necessary to protect the legitimate interests of the administrator and to settle relations arising from the termination of membership (archiving, etc.). BLAŽEK PRAHA as is also authorized to process personal data based on the fulfillment of a legal obligation in order to fulfill the obligations imposed on BLAŽEK PRAHA as by public law regulations.

BLAŽEK PRAHA as is also entitled to process personal data on the basis of the consent granted to the processing of personal data. BLAŽEK PRAHA as is entitled to process personal data for the purposes expressed in the consent, which are the implementation of marketing activities, the creation of surveys, analyses and business statistics regarding purchasing behavior and the sending of business communications or invitations to events organized by BLAŽEK PRAHA as. Analyses related to purchasing behavior and behavior within the Blažek Club consist primarily in the evaluation of customer data with the aim of appropriately adapting the product offering and improving the quality of the services provided, including profiling. Profiling is understood as the processing of personal data in the form of an evaluation of certain personal aspects, such as personal preferences, interests and purchasing behavior. The use of this procedure allows BLAŽEK PRAHA as to communicate effectively with customers, in particular by addressing them with an offer of goods and services that really interest them.

Based on the consent granted, BLAŽEK PRAHA as is also authorized to carry out automated processing of personal data (i.e. without any human intervention - for processing (i.e., using only automated means), such automated processing may have an impact on the offers and/or benefits intended for a specific customer. Such automated decision-making may be an integral part of some marketing processing of personal data, in particular when targeting special offers and benefits to customers. As a result, individual members of the Blažek Club may receive different benefits and offers, or some of the offers and/or benefits provided may be available only to selected customers.

Those interested in membership in the Blažek Club loyalty program and members of the Blažek Club are not obliged to provide their personal data to BLAŽEK PRAHA as. In such a case, however, BLAŽEK PRAHA as is entitled not to register the interested party to the Blažek Club, or to cancel membership in the loyalty program. BLAŽEK PRAHA as is entitled to cancel membership in the loyalty program even if consent to the processing of personal data is revoked. 3. Transfer of personal data

BLAŽEK PRAHA as may transfer personal data to third parties under the conditions set forth in generally binding legal regulations, if the transfer is necessary to ensure the proper functioning of the Blažek Club, to companies of the BLAŽEK PRAHA as group or to other persons under the conditions of the consent granted to the processing of personal data.

Based on the authorization of BLAŽEK PRAHA as, personal data may be processed by processors in the Czech Republic and abroad, to the extent strictly necessary for the specified purpose of processing. Processors according to the previous sentence are understood in particular to be persons authorized to manage the personal data database or to ensure its technical solution, as well as marketing agencies, call center operators and providers of postal and delivery services. 4. Rights in connection with the processing of personal data

In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation), you have the right to request information about the purpose of the processing of personal data, information about the scope of the personal data that are the subject of the processing and their categories, including information about the source from which the personal data were obtained, the planned duration of the processing, whether automated decision-making, including profiling, is taking place, and information about any recipients or categories of recipients of the personal data.

Every customer who has provided their personal data to BLAŽEK PRAHA as also has the right:

a) request from BLAŽEK PRAHA as access to your personal data, their correction, deletion and/or restriction of their processing;
b) the portability of personal data to another controller, if technically feasible;
c) if personal data are transferred to a third country, to be informed of this, as well as of the appropriate security guarantees that apply to the transfer; Upon your request, we will provide you with this information without undue delay in a structured, commonly used and machine-readable format. This right does not apply to personal data that are not processed automatically.
d) withdraw your consent to the processing of personal data at any time, including the right to have your personal data erased or anonymized without undue delay, unless there is another legal reason for processing them. Each customer, who has provided his/her personal data to BLAŽEK PRAHA as, has the following right: e) to object to the processing of personal data on the grounds of legitimate interest and the performance of a task carried out in the public interest or in the exercise of public authority;
f) object to the processing of personal data concerning him or her for direct marketing purposes, which includes profiling if it concerns direct marketing.
