Information memorandum of Blažek Slovakia s.r.o.

Let us inform you about how we at Blažek Slovakia s.r.o. process your personal data. The purpose of the Information Memorandum is to provide you with information mainly about what personal data we collect, how we handle it, from what sources we obtain it, for what purposes we use it, to whom we may provide it, where you can obtain information about your personal data that we process, and what your rights are in the area of ​​personal data protection. Blažek Slovakia s.r.o., ID number: 35917962, with its registered office at Einsteinova 18, 851 01 Bratislava, entered in the Commercial Register kept by the District Court in Bratislava I, Section Sro, Insert No. 34659/B (“hereinafter referred to as Blažek Slovakia, s.r.o.”), which is the operator of a network of its own operations (hereinafter referred to as “Blažek brand stores” or individually as “Blažek brand store”) and an online store (hereinafter referred to as “online store”) on the website www.blazek.en, is, as the administrator of personal data, obliged to protect your personal data and comply with the obligations arising from generally binding legal regulations regarding the protection of personal data.

1. Personal data

Blažek Slovakia, s.r.o. obtains personal data of customers within the scope of the order form and the registration form for the Blazek Club, available either online on the website of the company Blažek Slovakia, s.r.o., or in the printed form of the application for the Blazek Club, available in Blažek branded stores.

2. Blažek's authorization to process personal data

Blažek Slovakia, s.r.o. is authorized to process personal data based on the consent granted in order to ensure the proper functioning of the Blazek Club, based on the concluded contract in order to fulfill the rights and obligations of Blažek Slovakia, s.r.o. towards customers (such as data on orders, purchases and payment information) and the legitimate interests of Blažek Slovakia, s.r.o. (such as processing personal data for statistical purposes, informing customers about the activities of Blažek Slovakia, s.r.o., records of movements on the websites of Blažek Slovakia, s.r.o. (so-called cookies), preventing hacker attacks, etc.), during the duration of membership in the Blazek Club and after termination of membership for the period necessary to protect the legitimate interests of the administrator and to settle relations arising from the termination of membership (archiving, etc.), Blažek Slovakia, s.r.o. is also authorized to process personal data in order to fulfill a legal obligation in order to fulfill the obligations imposed on Blažek Slovakia, s.r.o. by public law regulations.

Blažek Slovakia, s.r.o. is further authorized to process personal data based on the consent granted to the processing of personal data. Blažek Slovakia, s.r.o. is authorized to process personal data for the purposes expressed in the consent, which are the implementation of marketing activities, the creation of surveys, analyses and business statistics related to purchasing behavior and sending business announcements or invitations to events organized by Blažek Slovakia, s.r.o.. Analyses related to purchasing behavior and behavior within the Blazek Club consist primarily in evaluating customer data with the aim of appropriately adapting the product offering and improving the quality of the services provided, including profiling. Profiling means the processing of personal data in the form of an evaluation of certain personal aspects, such as personal preferences, interests and purchasing behavior. The use of this procedure allows Blažek Slovakia, s.r.o. to communicate effectively with customers, in particular to address them with an offer of goods and services that really interest them.

Based on the consent granted, Blažek Slovakia, s.r.o. is further is authorized to perform automated processing of personal data (i.e. without any human intervention - exclusively automatic means are used for processing), while such automated processing may have an impact on offers and/or benefits intended for a specific customer. Such automated decision-making may be an integral part of some marketing processes of personal data processing, especially when targeting special offers and benefits to customers. As a result, individual members of the Blazek Club may receive different benefits and offers, or some of the provided offers and/or benefits may be available only to selected customers

Those interested in membership in the Blazek Club loyalty program and members of the Blazek Club are not obliged to provide their personal data to Blažek Slovakia, s.r.o. In such a case, however, Blažek Slovakia, s.r.o. is authorized not to register the interested party in the Blazek Club, or. cancel membership in the loyalty program. Blažek Slovakia, s.r.o. is authorized to cancel membership in the loyalty program even in the event of withdrawal of consent to the processing of personal data. The above procedure does not in any way deny customers the opportunity to purchase goods and use the services of Blažek Slovakia, s.r.o..

3. Transfer of personal data

Blažek Slovakia, s.r.o. may transfer personal data to third parties under the conditions set out in generally binding legal regulations, if the transfer is necessary to ensure the proper functioning of the Blazek Club, to Blažek Group companies or to other persons under the conditions of the consent granted to the processing of personal data.

Based on the authorization of Blažek Slovakia, s.r.o., personal data may be processed by processors in the Slovak Republic and abroad, to the extent necessary for the specified purpose of processing. Processors according to the previous sentence are understood mainly to be persons authorized to manage the personal data database or to ensure its technical solution, as well as marketing agencies, call center operators and providers of postal and delivery services. The list of processors to whom Blažek Slovakia, s.r.o. currently makes personal data available for further processing is available on the website of Blažek Slovakia, s.r.o..

4. Rights in relation to the processing of personal data

In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation) and Act No. 18/2018 Coll. on the Protection of Personal Data of 29 November 2017, you have the right to request information about the purpose of the processing of personal data, information about the scope of personal data that are the subject of processing and their categories, including information about the source from which the personal data were obtained, the planned processing period, whether automated decision-making, including profiling, is taking place, and information about potential recipients or categories of recipients of personal data.

Each customer who has provided Blažek Slovakia, s.r.o. with their personal data also has the right to:

a) request from Blažek Slovakia, s.r.o. access to their personal data, their correction, erasure and/or restriction of their processing;
b) the portability of personal data to another controller, if technically feasible;
c) if personal data are sold to a third country, to be informed of this, as well as of the appropriate security safeguards that apply to the sale;
Upon your request, we will provide you with this information without undue delay in a structured, commonly used and machine-readable format. This right does not apply to personal data that are not processed automatically.
d) withdraw your consent to the processing of personal data at any time, including the right to have personal data deleted or anonymized without undue delay, unless there is another legal ground for their processing;

Each customer who has provided Blažek Slovakia, s.r.o. with their personal data has the right to:
e) object to the processing of personal data for legitimate interest and the performance of a task carried out in the public interest or in the exercise of public authority;
f) object to the processing of personal data for direct marketing purposes concerning them, which includes profiling if it concerns direct marketing.

Receipt of electronic commercial communications can be canceled via the link contained in each individual commercial communication.

If you believe that your personal data is being processed without authorization, you can file a complaint with the supervisory authority of the public administration, which for the territory of the Slovak Republic is the Office for Personal Data Protection of the Slovak Republic (www.uoou.sk).
